    H5 - Navigating Medical and Behavioral Health Systems With Peer Recovery Support

    This workshop will focus on leveraging peer recovery support (PRS) to help patients feel connected and confident while navigating through multiple levels of care. We will share an example program design from a large medical and behavioral health system, in which PRSs work between multiple care settings to provide continuity for patients. We will encourage participants to identify an opportunity in their community where PRS could bridge the gap as patients transition between levels of care and consider what delivery model would be beneficial given their setting and resources available.

    Learning Objectives:

    • Explain how they would leverage peer support for patients transitioning to a new care setting.
    • Formulate solutions to administrative, cultural, and technological challenges with implementing peer support roles in health systems.
    • Demonstrate how to collaborate with multiple sites and teams to ensure continuity of patient care.
