E15 - Effective Implementation of Integrated Care in a Community Health Center Setting

This interactive session will explore practical considerations for effectively implementing an integrated care program, leveraging the experience of Community Health Center of the New River Valley leadership and staff. Quality improvement methods will be reviewed in the context of engaging staff in change and testing workflows before implementation. The impact leadership and culture have on implementation will be discussed. Billing and coding considerations will be highlighted. During the session, materials, including agendas and tip sheets, will be shared with participants.

Learning Objectives:

Participants will recognize key elements for implementing an integrated care program, including leadership, culture, and creating a high-performing team while considering billing and coding implications.
Participants will explore practical information and gain access to useful tools for implementing an integrated care program.
Through a use case, participants will discover emerging and best practices for implementing integrated care from a Community Health Center’s personal experience.
