    Collaborative Care is an evidence-based model to identify and treat patients with measurable behavioral health conditions in healthcare settings. Over a decade of experience supporting implementation of the Collaborative Care has taught us that there are key factors that make CoCM more likely to succeed. Any healthcare organization, practice or providers looking to implement or sustain CoCM should consider these elements. We will share data from primary care practices that shows where there are opportunities for improvement, and what practices should look for to ensure high fidelity, high quality sustainable CoCM.

    • Identify and analyze three key policy and provider-level challenges to CoCM implementation.
    • Demonstrate the use of performance and claims data to measure CoCM fidelity and patient outcomes.
    • Formulate actionable strategies for enhanced CoCM sustainability, including financial planning and interprofessional collaboration.
