    This presentation will discuss Recovery Housing (RH) as a model to bridge social and medical needs. Throughout the presentation, we will examine not only the services and models that yield recovery outcomes, but also how communities can foster a continuum of care that spans both social and medical needs, ensuring that individuals are supported in all dimensions of their recovery. Participants will explore how partnerships and services can be customized to community and resident needs and how communities can utilize collaborative approaches and blended funding models to meet health and social needs.
