MP21 - Increasing CCBHC Outcomes and Access: Implementing Data-driven Care Coordination and Brief Interventions
Monday, May 5, 2025
11:45 AM – 12:15 PM ET
Location: Hall BCD
This presentation showcases innovative practices from Merakey's Philadelphia Certified Community Behavioral Health Clinic (CCBHC) that enhance access to mental health services. Attendees will explore two key strategies: the utilization of health information exchange (HIE) data and the implementation of brief therapeutic interventions. The session reveals how a well-established community mental health center transformed its approach to providing comprehensive service delivery and improved care coordination. Participants will gain insights into transforming their own practices, learning from Merakey's adaptation to meet the evolving needs of the community while improving accessibility and outcomes in behavioral health care.
Learning Objectives:
Recognize the critical need for comprehensive, coordinated mental health care services and identify common barriers to accessibility. This understanding will be demonstrated through a post-presentation survey.
Explain how Merakey uses health information exchange, integrated records, and clinical dashboards to improve care coordination, with understanding assessed through a post-presentation survey.
Attendees will create a personalized care coordination improvement plan, applying workshop principles to their clinical settings for immediate, practical implementation of the strategies learned.
